Returning Family Enrollment

Returning families may enroll their students via our online form or by completing the Printable Enrollment Packet and sending it to us as outlined below.

Returning Family Enrollment Form

Use this form if you are re-enrolling students who have been enrolled with D1A in the past, even if it was not the last academic year. Please provide the information requested below for your returning students and for any new students you have to enroll.

*Students must be re-enrolled each school year.

ID Cards are not required. If you wish to receive ID cards from Daniel 1 Academy, they will be issued approximately two weeks from enrollment and will include two parent ID Cards and one card for each child enrolled.
